A thoughtfully designed 15-page fill-in digital workbook created from my personal experienceto help keep travel, documents, costs, appointments and support details in one convenient place for organizing the practical details involved in planning a procedure away from home.
The fill-in pages can be used to record information gathered through your own research and decisions, including details about a chosen provider and procedure, estimated costs, important documents, travel arrangements, accommodations, medications, appointments, support contacts and recovery plans.
This workbook is based on my personal experience and the organizational steps that helped me keep important information together. It does not conduct provider research, choose a provider or determine whether a procedure is appropriate for anyone.
The workbook also includes information about an independent international planning resource I personally used. This independent resource works with multiple doctors and hospitals and may provide information about available options while assisting with practical coordination such as appointments, transportation and local arrangements. Having this type of organizational support made my own planning experience feel more manageable.
